Ventilator Oh ventilator...

Terasa lama tak menulis dlm blog...sibuk sikit sehari dua ni. Hari ini ada kelas dengan MMed mengenai ventilator use in ED. Pening kepala juga dibuatnya..maklumlah bukannya expert sangat dalam hal ventilator. I think in emergency setting we have to know the basic setting in ventilator. Few things we have to know before use the ventilator based on mnemonic MOVEPAIRED :

M-mode of ventilator (SIMV/AC)
O-FiO2
V-tidal volume (6-8mls/kg)
E-PEEP (5-10)
P-plateu pressure <30
A-adequate sedation & analgesia
I-I:E atio
R-RR
E-ETT
D-Dead space

Few questions should be answered to understand the mechanical ventilator,

1. pressure vs volume cycle...the differences
2. mode of ventilator (CMV,AC,IMV)
3. SIMV vs AC -trigger factor
4. Permissive hypercapnia in reactive airway disease
5. Complications of mechanical ventilators (volutrauma,barotrauma,VILI etc)
